Stiff, Painful Hands: Possible Causes and Gentle Exercises That May Help

When your hands become stiff enough to make typing difficult, it is tempting to blame “getting older” or diabetic neuropathy. But stiffness and pain can come from joints, tendons, nerves, or several problems at the same time.
Diabetes raises the risk of several hand disorders—not just neuropathy—so a proper examination is more useful than guessing.
This article explains common possibilities, clues that distinguish them, gentle movements that may preserve function, and what to know about pain treatments—including DMSO.
It is educational, not a diagnosis. Progressive symptoms that interfere with typing deserve an appointment with a primary-care clinician, hand specialist, or occupational/hand therapist.
First, Notice the Pattern
Before the appointment, keep notes for a week. Record which fingers hurt, whether both hands are affected, how long morning stiffness lasts, and whether you have numbness, tingling, swelling, clicking, locking, or weakness.
Note whether symptoms wake you at night and whether you drop objects.
These details can point the evaluation in different directions:
- Burning, tingling, numbness, or electric pain suggests a nerve problem.
- Aching joints, swelling, reduced motion, or bony enlargements suggests arthritis.
- A finger that clicks, catches, or locks suggests trigger finger.
- Thick, tight or waxy skin with difficulty pressing the palms flat together may suggest diabetic stiff-hand syndrome.
- Pain at the base of the thumb during pinching, opening jars, or turning keys often fits thumb-base arthritis.
Possible Causes
1. Osteoarthritis
Osteoarthritis becomes more common with age and often affects the finger joints and the base of the thumb. Typical signs include pain with use, stiffness after rest, decreased motion, and sometimes swelling or bony bumps. Morning stiffness is often relatively brief—commonly less than 30 minutes. The National Institute of Arthritis and Musculoskeletal and Skin Diseases notes that exercise can reduce stiffness and improve flexibility and strength.
2. Diabetic Neuropathy
Diabetic peripheral neuropathy usually produces sensory symptoms such as burning, tingling, numbness, unusual sensitivity, or loss of feeling. It usually starts in the feet before reaching the hands. Neuropathy can contribute to weakness and clumsiness, but stiffness by itself is not the classic pattern. Good glucose management may help prevent or slow further nerve damage, although it may not reverse established damage.
3. Carpal Tunnel Syndrome
Diabetes increases the risk of carpal tunnel syndrome, in which the median nerve is compressed at the wrist. It commonly causes tingling or numbness in the thumb, index, middle, and thumb-side of the ring finger—but usually not the little finger. Symptoms may be worse at night, and the hand may feel weak or drop objects. A neutral-position wrist splint at night can help some people, but persistent numbness or weakness needs assessment because long-standing compression can permanently damage the nerve.
4. Diabetic Stiff-Hand Syndrome
Also called diabetic cheiroarthropathy or limited joint mobility syndrome, this diabetes-associated condition causes thickened or waxy skin and difficulty bending or fully straightening the fingers. One simple clue is the “prayer sign”: when you press your palms and fingers together, the fingers cannot make full contact. This is only a clue—not a self-diagnosis. Improving glucose control and receiving hand therapy may help preserve motion.
5. Trigger Finger
A thickened or irritated tendon can stop gliding smoothly, causing a finger or thumb to click, catch, or lock—often with tenderness near its base in the palm. Diabetes is a risk factor. Rest from forceful gripping, a clinician-recommended splint, gentle movement, a steroid injection, or occasionally surgery may be considered depending on severity.
6. Inflammatory Arthritis
Rheumatoid arthritis and other inflammatory diseases can cause warm, swollen, tender joints, often affecting both hands in a similar pattern. Morning stiffness lasting longer than 30 minutes is a clue. Early diagnosis matters because prescription treatment can reduce inflammation and prevent permanent joint damage.
7. Other Possibilities
Thumb-tendon irritation, gout, an old injury, medication effects, thyroid disease, vitamin deficiencies, nerve compression at the elbow, and a pinched nerve in the neck can also affect the hands. An examination—and sometimes blood tests, X-rays, ultrasound, or nerve-conduction testing—is needed to sort these out.
A Gentle Hand-Mobility Routine
Warm the hands first with a comfortably warm towel or warm water for five minutes. Check the temperature with an area of normal sensation; neuropathy can make burns easier to miss. Move slowly and never force a joint. Mild stretching is acceptable, but stop if you get sharp pain, increased tingling, or swelling.
Do the following once daily at first. Try five repetitions of each movement, holding the end position for three to five seconds. If the hands feel no worse later that day or the next morning, gradually work toward ten repetitions, once or twice daily.
1. Open and Close
Start with the wrist straight. Spread the fingers comfortably, then bring them together. Next, slowly form a loose fist with the thumb resting outside the fingers. Open again. Do not squeeze tightly.
2. Tendon-Glide Sequence
Move through these positions slowly:
- Fingers straight.
- “Tabletop”: bend only the large knuckles while keeping the fingers straight.
- “Hook”: keep the large knuckles fairly straight while bending the middle and end finger joints.
- Loose full fist.
- Return to straight.
This sequence helps the finger tendons glide. Skip any position that makes a finger catch or lock painfully.
3. Thumb-to-Finger Touches
Touch the thumb gently to the tip of the index finger, then the middle, ring, and little fingers. Make a relaxed “O” each time rather than pressing hard.
4. Thumb Across the Palm
With the palm facing you, move the thumb toward the base of the little finger, then return it to the side. Keep the movement comfortable.
5. Finger Lifts
Place the palm flat on a table. Lift one finger slightly, lower it, and proceed to the next. Do not force a finger flat if it will not straighten.
6. Gentle Wrist Motion
Rest the forearm on a table with the hand over the edge. Slowly bend the wrist up and down through a comfortable range, then gently turn the palm up and down. Keep the fingers relaxed.
Avoid hand grippers, hard balls, forceful stretching, or aggressive nerve-gliding exercises until the cause is clearer. Strength work can aggravate an inflamed joint, tendon, or compressed nerve. A certified hand therapist can tailor exercises and suggest larger pen grips, jar openers, keyboard changes, or voice-to-text tools.
Making Typing Less Painful
Keep the wrists straight rather than bent upward. Bring the keyboard close, use a light touch, and pause for one or two minutes every 20 to 30 minutes. Voice typing can reduce repetitive load while the cause is being investigated. If night numbness follows the carpal-tunnel pattern, ask whether a neutral wrist splint is appropriate; an incorrect or overly tight brace can make symptoms worse.
Medicines and Topical Treatments
Treatment should match the cause. A nerve-pain medicine will not correct a locked tendon, and an anti-inflammatory drug will not release a compressed nerve.
- Acetaminophen may reduce pain but does not reduce inflammation. It may be an option for some people, but liver disease, alcohol use, and other acetaminophen-containing products affect what is safe.
- Oral NSAIDs, including ibuprofen and naproxen, can reduce inflammatory pain but may cause stomach bleeding, raise blood pressure, interact with aspirin or other blood thinners, and worsen kidney function. Diabetes and known kidney concerns make it especially important to ask a clinician before using them.
- Topical diclofenac gel delivers less medicine to the bloodstream than oral NSAIDs and may help pain in superficial hand joints. It still has warnings and should be cleared with a clinician or pharmacist, particularly if you use aspirin, have kidney disease, take a blood thinner, or have heart or stomach problems.
- Prescription nerve-pain medicines, such as duloxetine, pregabalin, or gabapentin, may be considered for confirmed painful neuropathy. They can cause dizziness or sleepiness, and gabapentin or pregabalin dosing may need adjustment for kidney function.
- Steroid injections may help selected cases of trigger finger or carpal tunnel syndrome. They can temporarily raise blood glucose and must be weighed carefully in a person with diabetes.
What About DMSO?
Dimethyl sulfoxide, or DMSO, is promoted online as a topical treatment for arthritis and nerve pain. The evidence does not justify treating it as a proven remedy.
The National Center for Complementary and Integrative Health says there is too little research to determine whether DMSO helps osteoarthritis; one review of seven higher-quality studies found no significant pain reduction.
Reported effects include skin irritation, digestive upset, and a garlic-like taste, breath, or body odor.
The FDA-approved DMSO product is placed into the bladder by a clinician for interstitial cystitis. That approval does not establish that store-bought topical DMSO is safe or effective for stiff hands, arthritis, carpal tunnel syndrome, or diabetic neuropathy. Product purity and concentration may also be uncertain.
Because the skin carries substances into the body and because your underlying cause is unknown, do not experiment with topical or oral DMSO without discussing it with your clinician and pharmacist.
Never apply industrial-grade DMSO, place it on broken or infected skin, or combine it with other topical products.
When to Seek Medical Care
Arrange a routine appointment soon if symptoms are progressing, interfering with typing, waking you at night, or causing you to drop things. Bring your medication and supplement list and ask whether you need an A1c check, kidney-function testing, thyroid testing, hand X-rays, or nerve studies.
A referral to an occupational therapist or certified hand therapist can be especially useful.
Seek urgent care for a hand that suddenly becomes red, hot, severely swollen, or extremely painful; a wound, drainage, fever, or spreading redness; sudden inability to move a finger; or a cold, pale, or blue hand.
Call emergency services for sudden one-sided hand or arm weakness or numbness accompanied by facial droop, speech trouble, severe dizziness, or another possible stroke symptom.
The Bottom Line
Diabetic neuropathy is possible, but progressive stiffness and pain may fit arthritis, tendon disease, carpal tunnel syndrome, or diabetic stiff-hand syndrome more closely. Gentle movement can preserve mobility, but it should not replace diagnosis. Start softly, avoid forceful gripping, make typing easier, and have the hands examined before relying on DMSO or adding pain medicines.
